As with any mission trip there are necessary documents that need to be filled out. Please print and fill out the documents for your age group. All documents should be returned to your Mission Trip Leader. Please note that several forms will need to be notarized. There are several notary publics available to you at Hillside UMC. Contact the church office to insure that one is in the building when you stop by.
